Case Manager, Adult ID/DD

Are you highly organized and passionate about supporting adults with intellectual or developmental disabilities?

Bachelor's degree in a human services field required, such as psychology, social work, special education, counseling, or human development.

Schedule: The hours for this position are Monday-Friday 8:00am-5:00pm and a work from home option is available after an initial training period and meeting performance goals.

Staring Pay: $21.08/hour, with scheduled increases. Additional compensation commensurate with experience.

Case Managers in our Intellectual Disabilities (ID) or Developmental (DD) Disabilities Care Coordination program assist adults in accessing needed supports essential for living in the community and developing their desired lifestyle. Our program is responsible for coordination of resources and support designed to facilitate independence and stability. Activities include assessment, planning, linking, and monitoring services; crisis prevention and intervention services; and hospital and state facility admission and discharge planning activities.

A Glimpse of Care Coordination

Case Managers are essential for ensuring that individuals receive services that help them live the life they want. A typical week usually consists of phone calls, meetings, collaborating with other agencies in the community, home visits, and monitoring satisfaction of services which consists of visits with the individual, family members, and/or providers. Case Managers' schedules are self-driven and subject to change at times; being organized and having the ability to adapt to changing situations are keys to success.

Qualifications:

  1. Bachelor's degree in a human services field (psychology, social work, special education, human development, or other human services field) is required by state regulations.
  2. Experience working with adults with individual or developmental disabilities is a plus!
  3. Background Check: Candidates who are offered a position are required to undergo a background check and drug screen.
  4. Safe driving record: This position requires transporting clients and driving agency vehicles. Candidates must have a good driving record, which is defined as no more than two moving violations in the last three years and no criminal moving violations in the last five years.
