Hiring Range: $19.56/hour-$24.45/hour
Project LINK provides specialized substance use case management and counseling for pregnant women, mothers and their infants, and women of childbearing age.
The Project LINK Women's Services Coordinator will support the LINK Director and Managers in strengthening cross-agency collaboration and enhancing coordinated care efforts. This position will help establish and maintain a centralized service hub that bridges multiple systems of care, including hospitals, correctional facilities, OB/GYN practices, social services agencies, and community-based behavioral health providers. By formalizing and reinforcing these partnerships, the Women's Services Coordinator will help eliminate service fragmentation, ensure warm handoffs between agencies, and promote a holistic, woman-centered approach to care.
The LINK Women's Services Coordinator in Project LINK will identify, recruit and engage intravenous and or opioid use disorder individuals, to include pregnant and postpartum women to obtain substance use disorder and/or medication assisted treatment services. The Women's Services Specialist will also assist with referrals and linkages to SUD/MAT services, and will provide the initial linkage to provide successful enrollments in SUD/MAT program(s). The Women's Service Coordinator position will perform various case management services for the client including, but not limited to outreach, treatment planning, linkage, monitoring, advocacy, supportive counseling, crisis intervention, daily living support, and medication supervision. Some functions of this position will occur within local correctional facilities, local hospitals, local Human/Social Services offices, other community settings and BRBH CSB. In addition, the Women's Services Specialist will participate in co-facilitating Intensive Outpatient Treatment groups.
The schedule for the position is generally Monday-Friday 8:30am-5:00pm. No weekends or on-call!
