Intellectual Disabilities (ID) Care Coordinator

Care Coordinators are essential for helping adults with intellectual disabilities (ID) live their desired lives. A typical week usually consists of phone calls, home visits, and meetings with the individual, family members, and community providers. All services are documented according to state regulations.

You will work with the individuals on your caseload to find the right resources and supports to help them:

  • Live in the community: Find a place to live that meets their needs to facilitate independence and stability
  • Learn new skills and get supports: Create individualized service plans, connect them with helpful services, and monitor progress
  • Stay safe: Provide support during challenges, including crisis prevention/intervention and hospital facility admission and discharge planning

Qualifications:

  1. One year of experience in a human services field (e.g., group homes, behavioral health, social services).
  2. Education required is High School Diploma or GED
  3. Safe Driving Record: This position requires transporting clients and driving agency vehicles. Candidates must have a good driving record, which is defined as no more than two moving violations in the last three years and no criminal moving violations in the last five years.
  4. Background Check and drug screen
