Case Manager, ID/DD Medication Maintenance

The provision of case management services is designed to assist an individual in accessing needed medical, psychiatric, social, educational, vocational, residential, and other supports essential for living in the community and meeting basic needs. Individuals in the Medication Maintenance program are typically stable and have stepped down to medication only services from other higher levels of care. This position is Grant funded.

Starting Pay: $20.08/hour. Additional compensation commensurate with experience.

Qualifications:

  • Education: Bachelor's degree required.
  • Experience: One (1) year experience working with individuals with developmental disabilities.
  • Licensure/Certifications: Qualified Mental Health Professional (QMHP) or ability to meet requirements within one year under supervision preferred but not required.
  • Background Check: Candidates who are offered a position are required to undergo a background check and drug screen.
  • Safe driving record: This position requires transporting clients and driving agency vehicles. Candidates must have a good driving record, which is defined as no more than two moving violations in the last three years and no criminal moving violations in the last five years.
