Case Manager Discharge Planner, Forensic Jail Services

  • Starting Pay: $20.08/hour, with scheduled increases. Additional compensation dependent on experience.
  • Day schedule: Monday - Friday, day shift.

As a Case Manager Discharge Planner, you will assist incarcerated adults diagnosed with serious mental illness or co-occuring disorders in planning their supports for living in the community upon release. Responsibilities include:

  • Coordinating discharge planning
  • Linking with community services
  • Planning to meet identified needs
  • Making referrals for housing, employment, educational, and other resources
  • Scheduling follow-up appointments for treatment services, medication evaluations
  • Connecting individuals with case management and other related services
  • Developing relationships with local organizations

What we are looking for:

The successful candidate will have knowledge of counseling and the criminal justice system, passionate about working with incarcerated adults, and be comfortable working independently in a controlled environment.

Qualifications:

  • Education: Bachelor's degree in an acceptable human services field is required.
  • Safe Driving Record: This position requires transporting clients or driving an agency vehicle. Candidates must have a valid driver's license with a safe driving record (no more than two moving violations in the last three years AND no criminal violations in the last five years).
  • Background Check: Candidates who are offered a position are required to undergo a background check and drug screen that meet requirements of local detention facilities.
