Case Manager, Discharge Planner

The Case Manager Discharge Planner (Intercept 2) provides discharge planning and assessments, comprised of assessing individuals in the jail setting within 48 hours to determine if individuals would be appropriate for early release, case management, and other related services for persons with Serious Mental Illness (SMI), including Co-occurring Disorders, being released from the Roanoke City Adult Detention Center, and Roanoke County Jail who are either current BRBH clients or eligible for such services through BRBH. (MH Rural Jail Diversion funds/BHD Intercept 2 grant)

Starting Pay: $20.08/hour. Additional compensation commensurate with experience.

Requirements:

  • Education: Bachelor's degree in an acceptable human services field is required or bachelor in other field with minimum 90 hours of human services field classwork and CSAC-Supervisee eligible or Board of Counseling Certified Substance Abuse Counselor (CSAC) eligible.
  • Experience: One year preferred, but not required, of substance use related direct experience providing services to individuals with a diagnosis of substance use disorder, or with co-occurring diagnoses of substance use disorder and mental illness.
  • Licensure/Certifications: Qualified Mental Health Professional (QMHP) preferred. Substance Use Disorder Case Management (SUD CM): must meet requirements for qualification.
  • Safe Driving Record: Candidates must have a valid Virginia driver's license with a safe driving record (no more than two moving violations in the last three years and no criminal violations in the last 5 years.)
  • Background Check: Candidates that are offered a position are required to undergo a background check and drug screen.
