Children's Case Manager

Case Managers coordinate care to assist children/adolescents with a mental health diagnosis and their families in accessing supports essential for living in the community and meeting basic needs. A Day in the Life of a Case Manager:

Our Case Managers support children/adolescents who have a mental health diagnosis and their families. The Case Manager begins their services by developing a comprehensive plan of care for each of their assigned clients. The Case Manager then begins providing case management services based off of their client's identified needs. The highlight of a Case Manager's day is when a client and their family are able to overcome a barrier.

A typical day usually consists of (but not limited to!):

  • Phone Calls to assess needs, plan services, and coordinate care
  • Office and school meetings
  • Home and school visits
  • Participating in Doctor appointments
  • Collaborating with other community agencies
  • Educating the family about available support

In addition to the creation and implementation of individualized support plans, it is the Case Managers responsibility to ensure that the needs of their clients are being met, while maintaining all required documentation. 


Minimum Requirements:

1.  Degree: Bachelor's degree in a human services field (psychology, social work, special education, sociology, human development, etc).

2.  Experience: This position is entry-level. Clinical experience with children/adolescents is preferred.

3.  Good Driving Record: This position may occasionally require transporting clients or driving an agency vehicle. Candidates must have a valid Virginia driver's license with a safe driving record (no more than two moving violations in the last three years AND no criminal violations in the last five years).
