Position Summary/Responsibilities
The Support Coordination Lead plays a critical role in ensuring ID/DD Waiver clients maintain eligibility for essential services in compliance with DMAS, DBHDS Licensing, the Office of Human Rights, and the DOJ Settlement Agreement. This position works closely with the individual's primary case manager, families, and community waiver providers to complete annual reassessments and shared planning. The Support Coordination Lead ensures that all exceptional and extensive medical and behavioral support needs are accurately reflected in each individual's annual person-centered plan.
Key Responsibilities:
- Collaborate with primary case managers, individuals, families, and community waiver providers to complete annual service reassessments.
- Lead shared planning processes to ensure comprehensive support for clients.
- Document and ensure that all exceptional medical and behavioral support needs are included in the individual's annual person-centered plan.
- Maintain compliance with all relevant regulations and standards, including DMAS, DBHDS Licensing, Office of Human Rights, and DOJ Settlement Agreement requirements.
Hiring Range:
The anticipated base salary for this position is $52,175.95-$65,219.94. Actual compensation offered within this range will be determined based on the candidate's experience, education, qualifications (knowledge, skills, and abilities), geographic location, complexity of role, and other relevant business considerations. We kindly ask that candidates do not discuss compensation until after an offer of employment is extended.
Requirements
- Education: Bachelor's degree in education, social work, psychology or human services related field is required.
- Experience: Minimum of two (2) years of experience completing and implementing Person Centered Individual Support Plans for clients receiving ID/DD Waiver Services is preferred.
- Background Check: Fingerprints, State and FBI criminal record reports, drug test, and central registry report will be required upon selection.
- Safe Driving Record: This position requires transporting clients or driving agency vehicles. Candidates must have a good driving record, which is defined as no more than two moving violations in the last three years and no criminal moving violations in the last five years.
Special Work Conditions
While performing the duties of this job, the employee is regularly required to stand, sit, walk and bend. There may be an occasional need to crawl, climb or reach above the head. The employee regularly uses hands for grasping and manipulating objects with one or both hands. The employee occasionally lifts, carries and/or moves up to 10 pounds. Specific vision abilities required by this job include close vision. Specific hearing required by this job includes normal and soft tones. The ability to distinguish smells will be necessary. Local travel of up to 5% is required. Occasional travel outside of local area may be required. These functions may be performed with or without reasonable accommodations.
