Position Summary/Responsibilities

The Case Manager Medication Maintenance will track and coordinate care for clients with mental health and co-occurring substance use and/or developmental disabilities. Clients in this program are typically stable and have stepped down to medication only services from other higher levels of care. Staff will ensure clients receive necessary and applicable services, accepting and closing cases as needed. Will also staff and coordinate transfers to higher levels of care as needed and document transitions in the electronic health record. Will help ensure that access to medication as prescribed is maintained and help track those that need yearly primary healthcare screening and referrals to outside providers. Will assist with gaining access to outside healthcare records to encourage integrated healthcare efforts. (Funding from STEP-VA Primary Care Screening)

Hiring Range

The anticipated base salary for the therapist position is $40,404 to $45,775. Actual compensation offered within this range will be determined based on the candidate's experience, education, qualifications (knowledge, skills, and abilities), geographic location, complexity of role, and other relevant business considerations. We kindly ask that candidates do not discuss compensation until after an offer of employment is extended.

Requirements

  • Education: Bachelor's degree required.
  • Experience: Two (2) years of case management experience with population diagnosed with severe mental illness is preferable; previous work experience in a medical or behavioral healthcare setting very helpful.
  • Licensure/Certifications: Qualified Mental Health Professional (QMHP) or ability to meet requirements within one year under supervision preferred.
  • Background Check: Fingerprints, State and FBI criminal record reports, drug test, and central registry report will be required upon selection.
  • Safe Driving Record: This position requires transporting clients or driving agency vehicles. Candidates must have a good driving record, which is defined as no more than two moving violations in the last three years and no criminal moving violations in the last five years.

Special Work Conditions

While performing the duties of this job, the employee will often sit, stand, walk and bend. There will be occasion to stoop, climb and reach above their head. Use of hands for placing and pulling objects. The employee occasionally lifts up to 25 pounds and or carries up to 25 pounds and pushes or pulls up to 50 pounds. Specific vision abilities required by this job include close vision (computer). Specific hearing required by this job includes normal tones. Travel up to 1%. Travel is local. These functions may be performed with or without reasonable accommodation.
