Position Summary/Responsibilities

Case Managers coordinate care to assist children/adolescents with a mental health diagnosis and their families in accessing supports essential for living in the community and meeting basic needs. A Day in the Life of a Case Manager:

Our Case Managers support children/adolescents who have a mental health diagnosis and their families. The Case Manager begins their services by developing a comprehensive plan of care for each of their assigned clients. The Case Manager then begins providing case management services based off of their client's identified needs. The highlight of a Case Manager's day is when a client and their family are able to overcome a barrier.

  • A typical day usually consists of (but not limited to!):
  • Phone Calls to assess needs, plan services, and coordinate care
  • Office and school meetings
  • Home and school visits
  • Participating in Doctor appointments
  • Collaborating with other community agencies
  • Educating the family about available support

In addition to the creation and implementation of individualized support plans, it is the Case Managers responsibility to ensure that the needs of their clients are being met, while maintaining all required documentation.

Hiring Range:

The anticipated base salary for this position is $43,022.07 to $45,890.21. Actual compensation offered within this range will be determined based on the candidate's experience, education, qualifications (knowledge, skills, and abilities), geographic location, complexity of role, and other relevant business considerations. We kindly ask that candidates do not discuss compensation until after an offer of employment is extended.

Requirements

  • Education: Minimum of a high school diploma. An Associate's or Bachelor's degree in a human services field preferred.
  • Experience: Clinical experience in human services with children/adolescents/emerging adults is not required but is preferred.
  • Licensure/Certifications: Qualified Mental Health Professional (QMHP) preferred.
  • Background Check: Fingerprints, State and FBI criminal record reports, drug test, and central registry report will be required upon selection.
  • Safe Driving Record: This position requires transporting clients or driving agency vehicles. Candidates must have a good driving record, which is defined as no more than two moving violations in the last three years and no criminal moving violations in the last five years.

Special Work Conditions

While performing the duties of this job, the employee will often sit, stand, walk, bend, stoop, crawl, climb and reach above their head. Use of hands for placing and pulling objects. The employee frequently lifts and or carries up to 25 pounds and pushes or pulls up to 150 pounds. Specific vision abilities required by this job include close vision (computer work). Specific hearing required by this job includes normal tones. Travel up to 15% and is local. These functions may be performed with or without reasonable accommodations.
