Regional Dementia & Geriatric Specialist

The schedule for this position is Monday-Friday; day shift. No weekends!

Pay Range: $54,144.86-$81,217.29/annually

The Geriatric & Dementia Specialist provides Regional project management and coordination for the development and implementation of regional initiatives related to increasing access to, and obtaining quality services for geriatric individuals and or individuals with a dementia related diagnosis at risk for hospitalization and/or in need of stepdown services from state hospitalization. This position requires strong interpersonal, relationship building, and collaboration skills to work with community providers, private hospitals and state psychiatric facilities, playing a key role in addressing the current bed capacity crisis. Region served includes (3a): Catawba State Hospital and Blue Ridge Behavioral Healthcare (BRBH). Additional duties include:

  • Collaborates closely with DBHDS Older Adult Specialist, Region 3 Geriatric Team, Executive Leadership at the CSB, Social Services, various emergency services teams, emergency room staff, Crisis Stabilization Units, and 23 hour observation units, and discharge planning staff.
  • Establishes strong working relationships with community Assisted Living/Nursing Home providers in the region will be vital to success.
  • Development of community-based infrastructure partnerships for a growth in community placement opportunities will be an ongoing goal of this position.

This position requires the flexibility in daily schedule planning to accommodate potential short time frames of commitment hearings and other meetings across a large catchment area, and involves significant local travel.

Minimum Qualifications:

  • Education: Bachelor's degree in a human services field required. Master's degree in a human services field preferred.
  • Experience: Three (3) years' of experience working with older adults and/or individuals living with dementia.
  • Background Check: Candidates will be required to submit to a criminal background check and drug screen.
  • Safe Driving Record: This position may require transporting clients or driving an agency vehicle. Candidates must have a valid Virginia driver's license with a safe driving record (no more than two moving violations in the last three years and no criminal violations in the last five years).
