The provision of case management services is designed to assist an individual and their family in accessing needed medical, psychiatric, social, educational, vocational, residential, and other supports essential for living in the community and meeting basic needs. Individuals in the Medication Maintenance program are typically stable and have stepped down to medication only services from other higher levels of care (MHI grant funded).

Entry Level Pay $19.81/hour. Additional compensation commensurate with experience.

Minimum Requirements:

• Education: Minimum of a high school diploma. An associate's or bachelor's degree in a human services field preferred.
• Experience: One (1) year of case management experience with population diagnosed with serious emotional disturbances/serious mental illness is preferable, but not required.
• License/Certifications: Qualified Mental Health Professional (QMHP) or ability to meet requirements within one year under supervision preferred but not required.
• Safe Driving Record: Candidates must have a safe driving record (no more than two moving violations in the last three years and no criminal violations in the last five years) and have or be eligible to obtain a VA Driver's license.
• Background Check: Candidates who are offered a position are required to undergo a background check and drug screen.
