Forensic Services Case Manager (MAT Grant)

Do you have a passion for helping those in recovery from substance abuse? 

This Case Manager position provides services for clients in the local jails who are struggling with an Opioid Use Disorder or Alcohol Use Disorder who may benefit from participation in Medicated-Assisted Treatment (MAT). The Case Manager educates inmates about MAT services and helps them begin the medication before leaving the jail as well as during the transition back into the community. This position is funded by the Medicated-Assisted Treatment Grant.

Additional activities include:

  • Providing education to jail staff about MAT services
  • Discharge planning services
  • Coordination of care with MAT medical provider
  • Attending court
  • Connecting clients to available resources
  • Providing general case management services in the community

The successful candidate will have knowledge of available community resources and the criminal justice system and will be comfortable working both independently in a controlled environment and as an integral part of a team.

The schedule for this position is 8 hour days five days a week. It is a daytime position with the ability to flex time.

Entry Level Pay: $19.81/hour. Additional compensation commensurate with experience.

Minimum Qualifications:

  • Education: Bachelor's degree in an acceptable human services field.
  • Experience: One (1) year of substance abuse related experience providing direct services to individuals with a diagnosis of mental illness or substance abuse.
  • Safe Driving Record: Candidates must have a good driving record (no more than two moving violations in the last three years AND no criminal violations in the last five years (including DUI).
  • Background Check: Candidates must be able to pass background checks, including fingerprinting and those required by the local jails.
