DD Intake Specialist

Great Schedule: The schedule for this position is 8:00 AM - 5:00 PM with one hour lunch, Monday - Friday.

Starting Pay: $21.08. Additional compensation commensurate with experience.

The Developmental Disabilities Intake Specialist would be responsible for monitoring and coordination of DD Eligibility Determination and Intake Process. This position will serve as a liaison between the agency and those interested in DD case management services. Other duties include:

  • Serve as the main contact for people seeking case management services.
  • Coordinate with the DD Waiver Waitlist Specialist and DDACC Director to determine eligibility for the Waitlist.
  • Provide case management services which include activities designed to assist an individual diagnosed with a Developmental Disability in accessing needed medical, psychiatric, social, educational, vocational, residential, and other supports essential for living in the community and in developing his or her desired lifestyle.
  • Basic responsibilities include assessment, planning, linkage/referral, and monitoring.

Qualifications:

  • Education: Bachelor's degree in a human services field (i.e. sociology, social work, special education, rehabilitation counseling, nursing, or psychology)
  • Experience: A minimum of one (1) year related experience providing DD/ID support coordination services.
  • Background Check: Candidates who are offered a position are required to undergo a background check and drug screen.
  • Safe driving record: This position requires transporting clients and driving agency vehicles. Candidates must have a good driving record, which is defined as no more than two moving violations in the last three years and no criminal moving violations in the last five years.
