Case Manager MH Child, Project LINK

  • Starting Pay: $20.08/hour, with scheduled increases. Additional compensation dependent on experience.
  • Day schedule: Monday - Friday, day shift.

The Child Mental Health Case Manager coordinates care to empower pregnant women and mothers with substance use disorders to access supports for their children at risk for serious emotional disturbance. Efforts are focused on assessing families' needs and strengths, linking children with community services, monitoring children's' response to treatment, and ensuring quality of care is provided. All Case Management Services are sensitive to cultural values and preferences. This position is funded by the Pregnant and Postpartum (PPW) Project LINK Grant.

A Day in the Life of a Case Manager:

The Case Manager begins their services by developing a comprehensive and individual plan of care for each of their assigned clients. The Case Manager then provides case management services based off of their client's identified needs. The highlight of a Case Manager's day is when a client and their family are able to overcome a barrier.

A typical week usually consists of (but not limited to!):

  • Phone Calls to assess needs, plan services, and coordinate care
  • Office, home and community visits to meet client needs
  • Co-facilitating substance use disorder groups 2-3 times/week with therapists
  • Parenting/Family Education
  • Assistance in developing individual and family centered plans
  • Support with regular primary care, dental and therapy visits
  • Information on parenting skills and support with problem solving
  • Assistance with creating safe and healthy environment, developing healthy discipline, setting boundaries, structure and limits
  • Conducting regular developmental screenings to make sure children are meeting their milestones
  • Assistance with building family relationships and secure attachment in cases where this may have been interrupted
  • Collaborating with other community agencies

In addition to the creation and implementation of individualized support plans, it is the Case Managers responsibility to ensure that the needs of their clients are being met, while maintaining all required documentation.


Minimum Qualifications:

  • Education: Bachelor's degree in a human services field or a bachelor's degree in another field with at least 90 hours of human services field classwork.
  • Safe Driving Record: This position requires transporting client's or driving an agency vehicle. Candidates must have a valid Virginia driver's license with a safe driving record (no more than two moving violations in the last three years AND no criminal violations in the last five years).
  • Background Check: Candidates will be requires to submit to a fingerprint criminal background check and drug screen.
  • Experience working with individuals with a substance use diagnosis is a plus!
  • Licensure/Certifications: Eligible for supervision as a Certified Substance Abuse Counselor (CSAC)
