Case Manager, Child & Family

Case Managers coordinate care to assist children/adolescents with a mental health diagnosis and their families in accessing supports essential for living in the community and meeting basic needs. If you love working with families, being out in the community, and seeing change, CYFS MH Case Management is for you! 

Schedule: Monday - Friday, days with some late evenings to accommodate family needs. Once training is completed, a remote work schedule is available.

A Day in the Life of a Case Manager:

Our Case Managers support children/adolescents who have a mental health diagnosis and their families. The Case Manager begins their services by developing a comprehensive plan of care for each of their assigned clients. The Case Manager then begins providing case management services based off of their client's identified needs. The highlight of a Case Manager's day is when a client and their family are able to overcome a barrier.

A typical day usually consists of (but not limited to!):

  • Phone Calls to assess needs, plan services, and coordinate care
  • Office and school meetings
  • Home and school visits
  • Participating in Doctor appointments
  • Collaborating with other community agencies
  • Educating the family about available support

In addition to the creation and implementation of individualized support plans, it is the Case Managers responsibility to ensure that the needs of their clients are being met, while maintaining all required documentation.

Starting Pay: $19.81/hour. Additional compensation commensurate with experience.


Requirements:

  • Education: High School Diploma Required. Associate's or Bachelor's degree in a human services field is a plus.
  • Experience: Clinical experience with children/adolescents is a plus!
  • Safe Driving Record: This position may occasionally require transporting clients or driving an agency vehicle. Candidates must have a valid Virginia driver's license with a safe driving record (no more than two moving violations in the last three years AND no criminal violations in the last five years).
  • Background Check: Candidates who are offered a position must undergo a background check and drug screen.
