Case Managers coordinate care to assist children/adolescents with a mental health diagnosis and their families in accessing supports essential for living in the community and meeting basic needs. If you love working with families, being out in the community, and seeing change, CYFS MH Case Management is for you! The schedule for this position is Monday - Friday, days with some late evenings to accommodate family needs. Once training is completed and a case load is received, some remote work hours may be available.
Starting Pay: $20.08/hour. Additional compensation commensurate with experience.
A Day in the Life of a Case Manager:
Our Case Managers support children/adolescents who have a mental health diagnosis and their families. The Case Manager begins their services by developing a comprehensive plan of care for each of their assigned clients. The Case Manager then begins providing case management services based off of their client's identified needs. The highlight of a Case Manager's day is when a client and their family are able to overcome a barrier.

In addition to the creation and implementation of individualized support plans, it is the Case Managers responsibility to ensure that the needs of their clients are being met, while maintaining all required documentation.
